Team Leader Packet
This link provides all the information you will need to form a team and keep track of your team's fundraising efforts. Be sure to have each team member fill out an Individual Registration Form (available below) to be turned in with your team roster. Individual forms can be copied as needed.  

Individual Registration Form
Each Team Member will need one of these to keep track of donations they receive. Team Captains will turn in registration forms by April 20th. Donation Logs are due the day of the race.
